Individual mailers and questionnaire sheets can be picked up (during office hours) at the
or at your local county extension agent’s office. Questionnaire sheets can be downloaded from this website. Make sure that you have at least 100 grams (small sandwich bag) of soil sample to facilitate analysis. For savings on postal charges, multiple samples can be shipped together in one package. When shipping multiple samples, make sure there is a questionnaire sheet filled out for each sample and that your Sample Identification is labeled on each sheet and labeled in “permanent marker” on each sample bag.
